• ✔ Meets DBHDD and Medicaid waiver criteria
• ✔ Approved services and units in the Individual Service Plan (ISP)
• ✔ Service area availability and staffing alignment

Referral Pathways

Referrals typically come through Support Coordination agencies. Families may also contact us directly; we’ll coordinate with your Support Coordinator to confirm authorizations.
