Core Health Provider Services offers in-home elderly care that involves 24-hour supervision and management, health monitored by a registered nurse, nutritious meals and snacks served daily, assistance with medication management, housekeeping, laundry services, and personal care as well as activities of daily living. We respect our clients’ needs, and as much as possible, we want to give them a comfortable and worthwhile experience with us. With us, rest assured that you are handled with the utmost professionalism. We believe that our compassionate staff can accommodate you and give you the personalized services that you need. CHPS offers the following in-home care services:

Skill Nurses Services

Registered Nurses and Licensed Practical Nurses may also delegate Certified Nursing Assistants or CNA’s in performing much simpler healthcare-related tasks to our clients. What is emphasized in the way we provide our services is that it is totally uniquely created for every person.

You can rest your thought that your every need will be considered during the assessment process, for we believe that individuals are different and therefore should not be subjected to a one-size-fits-all type of treatment.

Using their specialized set of skills and expertise in their respective fields, our Nurses will guide you towards recovery and healing by engaging in the following:

• 1 - Assessing, observing, evaluating and managing care plans to ensure improvements in patient’s condition
• 2 - Wound care using assisted therapies
• 3 - Medication management, education and administration
• 4 - Tube feeding
• 5 - Blood sugar and pressure monitoring

Companion Services

Core Health Provider Services knows just how important it is to meet one’s emotional and social needs. Humans are innately born to be social beings. Especially with the elderly, they would highly appreciate the company of someone who would listen to both their woes and recollections of past achievements.

Aging is a normal consequence of life, but it does not have to be something that we dread to experience. Our companions and sitters will make you feel just that with the compassion they inculcate and incorporate in every action that they do.

To address your emotional needs, you will find our home care professionals doing a variety of things. We can plan games suitable for your age and condition, assist you with hobbies including handicrafts and gardening, or simply engage in conversation with you.

Sitting services may be offered round-the-clock or during specific periods of time only.

Personal Support services

Are you in need of continuing health care assistance following a hospitalization or you simply find it difficult to deal with your daily activities on your own?

Core Health Provider Services has the solutions you might be seeking. We know how problematic things can get, but our nurturing team of professionals will cater to your ever-specific need in a manner that you truly deserve.

Expect our professionals to sit down with you and your loved ones in planning and implementing the plan of care. Services may include assistance for bathing, grooming, dressing, nail and skin care, toileting, meal preparation, medical appointment assistance, medication management, and simple housekeeping.

To make our services truly accessible, we make them highly affordable for everyone by working with Georgia’s Medicaid Programs, namely the CCSP. These programs are specifically targeted to cater to the disabled, the low-income groups, and the elderly.

Private pay arrangements are also allowed along with private insurance plans. And since our schedules can be made flexible, ranging from 2 to 24 hours availability, you may contact us anytime or whenever the need arises.
