Core Health provider Services is a conglomeration of entities that provides services for individuals with Intellectual Developmental Disabilities, Elders, Seniors and Adults. The bedrock of the care provided is to afford the individuals choices, independence, and transition into the community. The support services range from a few hours a week to fully engaged support in the individual’s or family’s house. The central goal and objective remain a smooth transition from dependence to independence. Our approach is “Person-Centered”, supportive, and individualized to give them the tools to lead as normal a life as is practicable. An unwavering commitment and uncompromising dedication to caring for individuals with an intellectual and developmental disability is the cornerstone to Core Health Provider Services. Services are available within the Atlanta metro areas, including surrounding counties based on needs and location. To qualify, individuals must meet the Federal definition of Intellectual & Developmental Disability.
