Intellectual & Developmental Disabilities

CHPS provides superb care for the IDD populace. These services extend to diverse adults with IDD and it is an equal opportunity service-based care, with emphasis on “person-centered’ approach. Programs and services are designed with the individuals, their families, and coordinators in mind. CHPS promotes and facilitates the individual’s reasonable active involvement in their care and participation in their life’s choices. This clearly promotes self-reliance and confidence, as it equally facilitates a successful transition to independence and sustainable happy living.

Respite - In Home

Brief support is provided for those that may need it as a relief for family and/or for a variety of other reasons. For instance, CHPS’s respite program is available when families or other unpaid caregivers need relief or support for a relatively brief period or on day-to-day (overnight) basis, participants need relief or a break from a caregiver, structured, support and/or when relief is necessitated due to unforeseen circumstances and emergencies. Respite is typically brief and maybe in the individual’s home or out-of-home.

Individualized Community Access (CAI)

Individualized Community Access Services are provided to those either living at home with their family or in the community in their own homes. In either case, the goal is to support people to live as independently as possible by acquiring, retaining, or improving socialization, and adaptive skills for active community participation and independent functioning. Areas of assisting may include, but not limited to money management, teaching appropriate shopping skills, using public transportation, and teaching nutrition and diet information. As with all our services, we support people to become part of their community. We recognize that community inclusion and access is especially important for those living independently.

Community Access Group (CAG)

Core@CAG houses our Community Access Group and Prevocational Services, also referred to as “the Day Program”.

We serve adults with intellectual and developmental disabilities ages 18 and older. We are a community-access day program. This means we focus on building connections within the surrounding Atlanta community through community outings and events. We have many activities available on days when our clients do not go out into the community.

Our Prevocational Services prepares an individual for paid or unpaid employment. Our Day Program offers a wide variety of social, volunteer, and educational activities to help develop Individual skills and strengths. Different learning styles are utilized in areas of learning and training based on the Individual’s cognitive, motor and intellectual skills.

Supported Employment

Supported Employment Initiative helps adults with intellectual and developmental disabilities who want to work gain the skills they need to get a job and keep it. We also help individuals refine their skills and determine their interests to help them find the right fit for employment. We provide on-going support to ensure long-term success. In addition, we help employers and individuals with disabilities find each other and meet each other’s employment needs.

Core Health Independent Living

Core Health Independent Living is an individualized Community Residential Alternative living program enabling adults with disabilities to live as independently as possible in a 24-hour shared home. Our supported Living offers 24/7 support to three or four individuals in an apartment or single-family home in a nice neighborhood.

Transportation Encounter Services

One of the most essential components of transitional independent living and enhanced quality of life for the individuals we support is transportation. CHPS offers this service for a reasonably competitive fee. Transportation of the individuals is provided to and from various destinations and points, work, medical appointments, social functions, shopping, and recreational engagements. This service is contingent on the area and locale, and it is offered Monday through Friday from 8 a.m. – 6 p.m. Special provisions may however be reasonably accommodated. Drivers are on staff and may be available for field and group trips. Also, vehicles are handicap compliant.

Community Living Support (CLS)

For individuals who reside either in their family home or who live independently in their own home or apartment, we provide Community Living Services. Hours and types of support provided are based on the needs and desires of the individuals and/or their family members. Areas of support might include but are not limited to, assistance with meals, shopping, personal hygiene, community involvement, and teaching the skills related to activities of daily living.
